If you need to book an appointment, have a test or collect a medicine, start with the possible reason for an exemption: income and social circumstances, a medical condition, recognised disability, or a specific situation such as pregnancy. Each follows a different process, and none alone guarantees that every expense is covered.
This guide helps you identify the code to check, prepare documents, verify recognition with your local health authority (Azienda sanitaria locale, ASL) and obtain the appropriate prescription or referral. National codes E01–E04 use the required tax income assessment, not automatically ISEE. Medical conditions and disability require the applicable recognition and documentation; some procedures and additional exemptions depend on the Region.
What to check before booking
- Income
- E01–E04: specific conditions and tax income; ISEE is not a substitute
- Recognition
- Automatic lists or an income self-declaration; medical documentation and the ASL for conditions and disability
- Coverage
- Check the service, code and SSN arrangement: exemption does not make every treatment free
- Prescription
- The appropriate code must appear on the prescription or referral under the applicable procedure
- Validity
- Read your certificate: there is no single expiry date or renewal process for everyone
- Location
- Medicines, digital services and additional exemptions may follow regional rules
Section 1
Where to start: choose the relevant pathway
The ticket is a co-payment towards care provided through Italy's National Health Service (SSN). Before looking for a form, identify the reason for exemption and the type of service.
| Your situation | Who to contact first | What to check |
|---|---|---|
| Low income combined with the required age or social circumstances | Your doctor, regional service or local health authority (ASL) | The relevant E01–E04 code, eligibility and an active certificate; any required self-declaration. |
| A diagnosed chronic condition or rare disease | Specialist and ASL; a network centre for rare diseases | Accepted diagnostic documentation, recognition and covered services. |
| Recognised disability | The ASL, with the required assessment report | Category, code and scope of coverage; any reassessment. |
| Pregnancy or prevention | Doctor, family counselling clinic (consultorio) or screening programme | Covered services, clinical or timing conditions and the code on the referral. |
| A charge for a medicine | Pharmacist, doctor and regional information | Fixed charge, reference price, medicine class and applicable code. |
This guide concerns access to care through the SSN or the regional health service (SSR). A private appointment or a visit booked as libera professione does not become free simply because you hold an exemption. If you first need to clarify healthcare registration or entitlement, address that with your ASL.
Section 2
Income exemptions: codes E01, E02, E03 and E04
Low income alone is not enough: each national code combines specific conditions with an income test or a particular social benefit.
| Code | Who it concerns | Financial condition |
|---|---|---|
| E01 | People under 6 or over 65. | Total tax-household income no higher than €36,151.98. |
| E02 | Unemployed people and tax-dependent family members, subject to the recognised employment status. | Household income below €8,263.31; the threshold rises to €11,362.05 where there is a spouse, plus €516.46 for each dependent child. |
| E03 | Recipients of pensione sociale or assegno sociale and tax-dependent family members. | The specified social benefit is required: a pension that is simply low does not establish eligibility. |
| E04 | Recipients of a pensione al minimo aged over 60 and their tax-dependent family members. | Household income below €8,263.31; the threshold rises to €11,362.05 where there is a spouse, plus €516.46 for each dependent child. |
The thresholds have different boundaries: E01 allows income equal to the limit; for E02 and E04, income must be below the applicable limit. E01 is personal: one person's eligibility does not automatically exempt the other family members.
For E02, ask which employment status and evidence your local service requires. Lombardy's instructions also include people who have never worked, with a dichiarazione di immediata disponibilità (DID), a declaration of immediate availability for work, and the other requirements; ASP Palermo's instructions instead require previous work as an employee. Do not automatically equate unemployment, economic inactivity and looking for a first job.
Section 3
Tax income and ISEE: two different assessments
For E01–E04, use the income and household definitions required for the exemption. Do not enter your ISEE value or household-registration details without checking.
- Total income — reddito complessivo
- This is the relevant tax income, not your monthly take-home salary. Tuscany's instructions refer to the sum of total incomes before deductible expenses and specify that income from buildings and rent subject to cedolare secca must also be considered.
- Tax household — nucleo familiare fiscale
- This includes the person concerned, a spouse who is not legally separated and tax-dependent family members under the applicable rules. It does not necessarily match the people living together or the ISEE household.
- Reference year
- The income self-declaration under the MEF decree refers to the previous year. For a declaration relating to 2026, the income reference is therefore 2025; follow the official form for the dates and social circumstances to declare.
- ISEE
- This indicator combines income, assets and household composition under its own rules. It may be required for particular regional schemes, but does not automatically replace the national E01–E04 assessment.
Section 4
Automatic recognition and income self-declarations
First check whether your code is already recorded. If it is missing, check eligibility and the approved channel: absence from the list alone proves neither ineligibility nor entitlement.
Check your record
Ask your doctor or ASL, or use the official service identified by your Region or the electronic health record (Fascicolo sanitario elettronico) where available. Check the beneficiary, code, start date and expiry date on the certificate.
Check eligibility again
Sistema Tessera Sanitaria (Sistema TS), the national health-card system, uses tax and social-benefit information. An automatically assigned code does not replace checking your circumstances and the required income reference period.
Submit a self-declaration if needed
Use the form and channel authorised by your ASL or Region. Prepare identification, tax codes and the required details for each beneficiary. Declare only circumstances you can accurately attest, and keep the receipt or reference number.
Check the outcome
Verify that the certificate has been issued and recorded. A draft, a booked appointment or simply sending a request does not prove that the code can already be used.
Check the prescription or referral
Tell the prescriber about the relevant exemption and check that the code appears. Do not add or alter it yourself; report a missing code before receiving the service.
For 2026, ASP Palermo documents access to the citizen service on Sistema TS with SPID, CIE or TS-CNS: where applicable, it allows self-declarations, consultation and printing of certificates, or closure of an exemption. It also lists free help from participating CAF centres. This is a territorial example: it does not mean that every CAF or portal handles every exemption throughout Italy.
Declarations are checked. If you lose eligibility, do not keep using a code merely because it remains visible in the system: request closure or correction through the responsible service. Charges you should have paid may be recovered, alongside the consequences provided for untruthful declarations.
Section 5
Chronic conditions and rare diseases: diagnosis, ASL and coverage
A medical-condition exemption follows recognition of the condition and its covered services. A general diagnosis or medical report is not a code you can use yourself.
| Pathway | Documentation and recognition | Scope |
|---|---|---|
| Chronic conditions | A diagnosis certificate and medical documents accepted under the rules; submission to the responsible ASL for an exemption certificate. | Services specified for the condition and appropriate for monitoring, treatment and preventing deterioration; not every appointment or test. |
| Rare diseases | Certification from a network centre for a listed disease or group; an exemption application to the ASL. | Appropriate and effective services for treating and monitoring the diagnosed rare disease and preventing deterioration. |
| Suspected rare disease | A diagnostic suspicion raised by an SSN specialist and a pathway through centres in the network. | Diagnostic tests within the required pathway, including use of code R99 where applicable; not a citizen's self-declaration of a suspected condition. |
Obtain the appropriate certificate
Ask your specialist which document is needed for exemption. For rare diseases, identify the responsible network centre. For chronic conditions, if you already have an assessment report or clinical record, ask the ASL whether it is acceptable in your case.
Apply to the ASL
Follow local instructions for the office or digital channel, with identification, your health card and the required medical certification. Check any authorisation requirements if you act for someone else.
Read the exemption certificate
Check the code, start date, any expiry date and the services concerned. Rare-disease codes normally begin with R; chronic-condition codes identify the particular condition. Do not infer coverage from the disease name alone.
Use an appropriate referral
The doctor must apply the exemption to the relevant covered services. A new diagnosis, an updated list or a change in healthcare registration may require the ASL to check the record.
Tests to establish an initial chronic-condition diagnosis are not automatically exempt: the diagnostic pathway described for a suspected rare disease has its own conditions. The rare-disease pathway may also cover genetic investigations of family members needed to diagnose the patient, under the applicable conditions. This does not automatically exempt those relatives from charges for every other service.
Section 6
Disability: the recognised category matters
A disability assessment report and a ticket-exemption certificate serve different purposes. Take the required recognition documents to the ASL and check the code assigned.
| Code | Category | Practical check |
|---|---|---|
| C01 | Recognised civil disability of 100%, without indennità di accompagnamento, the attendance allowance. | Exemption for all outpatient specialist services within the LEA; check ASL recognition. |
| C02 | Recognised civil disability of 100%, with the attendance allowance. | Exemption for all outpatient specialist services within the LEA; check any reassessment date. |
| C03 | Recognised civil disability from 67% to 99%. | Exemption for all outpatient specialist services within the LEA; do not assume the same rules apply to every lower percentage. |
Other categories include work-related, service-related or war disability and particular recognised circumstances. Some provide broad coverage of specialist care within the LEA; others cover services related to the condition. Do not transfer the treatment of one code to another.
Prepare the accepted assessment report or certificate, identification and health card; ask the ASL whether you also need to submit the outcome of a reassessment. Recognition under Law 104 does not by itself amount to a national exemption code. Medicine rules must be checked separately.
Section 7
Pregnancy and prevention: specific services and conditions
Free care concerns specified services prescribed appropriately, or prevention programmes with their own criteria. It does not cover every requested test without distinction.
| Situation | Code or pathway | Limit to check |
|---|---|---|
| Preconception services | M00, for the services provided for. | Accepted services and the couple's or individual's circumstances; not a general exemption for any check-up. |
| Uncomplicated pregnancy | M followed by the pregnancy week, under the prescribing instructions. | Services and timing specified in the relevant list. |
| High-risk pregnancy or prenatal diagnosis under the specified conditions | M50, where the documented conditions apply. | Assessment and prescribing by the appropriate professional; no self-assignment. |
| Organised screening | The regional prevention pathway and the code required by the programme. | Eligible people, invitation, test and programme interval. A private test outside the pathway does not automatically become free. |
For pregnancy, start with your doctor, gynaecologist or consultorio: ask which referral is needed and whether any local registration is required. There is no single ASL certificate that everyone must request for every pregnancy-related service.
Other codes and conditions also apply to particular cancer-screening investigations. Ask the screening service or prescriber about the applicable age, intervals and pathway; describing a visit as preventive does not exempt it from payment.
Section 8
Full or partial exemption: specialist care and medicines
Always ask which charge is waived. Read “full” within the scope of the code and service, rather than as a promise of universally free care.
| Area | What to check | Mistake to avoid |
|---|---|---|
| SSN specialist appointments and tests | A valid code, an eligible service, appropriate prescribing and a correct referral; a public or accredited provider delivering the service under the SSN. | Assuming a private service or one outside your exemption is free. |
| Medical-condition exemption | The specified related services and your doctor's referral. | Using the condition's code for any other appointment. |
| Medicine co-payment | Regional rules, the applicable code and the medicine's reimbursement class. | Automatically applying an outpatient-care exemption to a medicine. |
| Difference from the reference price | Ask the pharmacist whether the charge is a price difference and whether it remains payable in your case. | Confusing this difference with the fixed regional ticket. |
| Class C medicines | Normally paid for by the patient; any exception needs a specific provision. | Assuming any ticket exemption makes every medicine free. |
An exemption may remove the charge for all services within its scope, or cover only particular services or parts of the cost. Even where a category is fully exempt from specialist-care charges, medicines, excluded services and other costs follow their own rules.
Lombardy, for example, distinguishes national codes from regional pharmaceutical exemptions. If you move to another Region or receive care outside your area of registration, ask beforehand how your code will be recognised and which charges still apply. Do not transfer regional thresholds or coverage from one place to another.
Section 9
Validity, renewal and loss of eligibility
The duration is determined by your certificate and the applicable rules. Do not rely on your health card's expiry date or a deadline found for another Region.
| Type of exemption | Check | Next step |
|---|---|---|
| Automatically assigned income exemption | The validity period and continued eligibility; the list may use information that needs updating. | If the code is missing or the details are wrong, ask the ASL whether to submit a self-declaration, correction or closure request. |
| Self-declared income exemption | The certificate's expiry date and local annual instructions; employment status where relevant. | Renew through the required channel if needed and if you still qualify. |
| Chronic condition or rare disease | Any minimum validity period, expiry date, diagnosis and update to the applicable list. | Ask in advance whether a new medical certificate or record check is needed; do not assume all condition-based exemptions expire annually. |
| Disability | The category, exemption certificate and any reassessment of the recognised disability. | Submit updates required by the ASL; do not infer validity or coverage solely from the date of an old assessment report. |
| Pregnancy or a prevention pathway | The period, service and conditions on the referral or set by the programme. | Check the appropriate code for the appointment: do not treat it as a permanent personal exemption. |
Territorial example: AUSL Romagna describes income-exemption validity from 1 April to 31 March; E01, E03 and E04 may be assigned automatically, while E02 requires an annual self-declaration. These instructions do not establish a universal national expiry date for every exemption.
If you start work, your social circumstances change or you discover that the declared income does not meet the requirements, contact the responsible service without waiting for expiry. Keep the previous certificate, proof of your communication and the new outcome. Ask how to handle existing referrals or scheduled appointments: do not assume a remaining period of eligibility.
Section 10
Regional differences: three practical examples
Regions may organise different channels and provide additional exemptions. These examples show which details to check locally; their rules cannot be extended to the whole of Italy.
| Area | Documented difference | What to do |
|---|---|---|
| Sicily — ASP Palermo | 2026 instructions on the TS service, SPID/CIE/TS-CNS and participating CAF centres; previous work as an employee is required for E02. | Check your certificate and your ASP's instructions, including renewal and employment-status documents. |
| Lombardy | E02 may also include people who have never worked, with a DID and the specified requirements. Regional codes provide distinct pharmaceutical coverage. | Use the regional page and check the code, scope and documents with the ASST/ATS or responsible service. |
| Tuscany | In 2026, E90, E91 and E92 concern specific employment circumstances and dependent family members, with ISEE no higher than €18,000; regional validity runs until 31 December 2026. | Check the individual category and where it can be used. Do not turn this ISEE threshold into a national E01–E04 requirement or assume an extension into 2027. |
To find the correct process, start on your Region's or ASL/ASP's website and search for “esenzioni ticket” together with the relevant reason. Check who can apply, the channel, documents, start date, expiry date, covered services and territorial validity. If a page gives no year or conflicts with a newer notice, ask the exemption office to clarify.
Section 11
Common problems: who can check them
Identify whether the problem concerns recognition, the referral or payment. Keeping the documents helps the right service investigate the relevant step.
| Problem | Useful check | Office or professional |
|---|---|---|
| The code is missing | Beneficiary, certificate, validity and registration; any request still being processed. | ASL or regional-service support. For income exemptions, ask whether a self-declaration is available. |
| The exemption appears, but your circumstances have changed | Income reference year and changes in work, household or social benefit. | ASL or the closure/correction channel; do not continue using it simply because it is visible. |
| The referral has no code | The referral and appropriate code before booking or receiving the service. | The prescribing doctor; the CUP booking service or provider for handling the appointment correctly. |
| The provider still requests payment | Whether the charge is a ticket, a service outside the exemption or privately provided care. | The CUP or provider's administrative office, with your referral and certificate. |
| The pharmacist requests payment | Regional charge, price difference, medicine class and the code used. | Pharmacist; if your record is wrong, doctor or ASL. |
| You have already paid for care you believe was exempt | Referral, certificate date, receipt and the local procedure's conditions. | ASL or provider for a check and any correction or refund: reimbursement is not automatic. |
| You receive a repayment demand | Whether the notice is genuine, the period disputed, the reason and the stated deadline. | The issuing office; a CAF or professional for tax information. Do not ignore the deadline or assume it is automatically suspended. |
If you cannot use the online channel, ask the ASL which offices or participating support organisations are available and whether authorisation to act for someone else is needed. Do not share passwords, PINs or one-time codes. Percorso Cittadino does not receive medical certificates, assessment reports, health cards or tax declarations, and does not decide your exemption entitlement.
Section 12
Documents and a reusable checklist
This list helps you prepare: the actual attachments are those required by the official procedure for your category.
| Pathway | Keep available | Do not assume |
|---|---|---|
| Everyone | Identification, health card/tax code and any previous exemption certificate; representation documents or authorisation where required. | The health card does not replace the exemption certificate or prove eligibility on its own. |
| Income | The official form, tax-household details and income for the required year; CU, 730 or Redditi documents and other information needed to reconstruct the figures; relevant pension or employment-status details. | These documents do not necessarily all need to be attached: follow the service's instructions. ISEE is needed only where the particular scheme requires it. |
| Medical condition | Accepted diagnostic certification and documents specified by the ASL; for rare diseases, certification from the network centre. | An arbitrary medical document or self-diagnosis is not sufficient. |
| Disability | The accepted assessment report/certificate and relevant updates or reassessment. | A general reference to “Law 104” does not identify the exemption on its own. |
| Pregnancy and prevention | Referral, professional instructions or programme invitation; documentation relevant to the pathway. | Do not assume there is a single national application for every service. |
Before the appointment or test
- I have identified the reason for exemption and checked the requirements using the source for my area.
- I know which income, household definition or medical document is required: I have not automatically substituted ISEE for income.
- I have checked the certificate: beneficiary, code, start date, expiry date and continued eligibility.
- The service is provided under the correct healthcare arrangement and falls within my exemption.
- The doctor has entered the appropriate code on the referral; I have clarified any remaining charges.
- I keep receipts and communications and know the channel for renewal, correction or closure.

Frequently asked questions
Quick answers to read alongside the code's conditions and the instructions for your area.
Does a low ISEE automatically qualify me for a ticket exemption?
No. National codes E01–E04 require particular age or social conditions and, where applicable, the total income of your tax household: ISEE does not replace that assessment. Some regional schemes use ISEE instead. Check the relevant code with your ASL rather than assuming eligibility from the certificate's value alone.
Does E01 also cover my family?
E01 applies to the person who meets both the age and income conditions. It does not automatically extend to the whole family. E02, E03 and E04 also cover tax-dependent family members under their respective conditions; each beneficiary must be correctly identified.
Can I apply for E02 if I am unemployed?
You must check both income and the employment status recognised in your Region. Territorial instructions differ: under its conditions and with a declaration of immediate availability for work (DID), Lombardy also admits people who have never worked; ASP Palermo requires previous work as an employee. Having no salary does not by itself establish eligibility.
How can I check whether my exemption is already active?
Check your certificate through the official service available in your area, through the electronic health record where provided, or ask your doctor or ASL. Check the code, beneficiary, start date, expiry date and continued eligibility. Before using a referral, check that it shows the appropriate code: the health card alone does not prove an exemption.
Does a chronic condition make all appointments free?
No. A chronic-condition exemption covers the services specified for that condition, subject to appropriate prescribing. Rare diseases have a different scope of covered services and a dedicated diagnostic pathway. The ASL recognises the exemption on the required documentation: you cannot choose a code yourself.
Does the exemption cover all medicines too?
Not automatically. Charges for specialist care, medicine co-payments and any difference from the reference price are separate items. The Region, exemption code, medicine class and specific conditions determine what remains payable. Ask the pharmacist to explain the charge; an exemption does not generally make class C medicines free.
Are exemptions always renewed on 31 March?
No. Validity and renewal depend on the type of exemption and territorial rules. Some income exemptions are assigned automatically; others require a self-declaration. Condition-based or disability exemptions can have different durations. Read your certificate and the ASL notice, and report loss of eligibility even before expiry.
Can I recover a ticket payment if the referral was missing the code?
This guide cannot guarantee an automatic refund. Before the appointment or test, ask your doctor and the provider to check the referral. If you have already paid, keep the referral, certificate and receipt, and ask the ASL or provider whether your case meets the local conditions for correction or reimbursement.
